What to Expect During Your First Visit to Family Care Chiropractic

If you are unable to access or print the forms at home, please arrive at least 15 minutes early to fill them out in the office.

The patient intake packet includes several brief forms designed to give us a full understanding of your health history, current symptoms and care preferences. You will be asked to provide:
- Contact and demographic information
- Medical history and past treatments
- Details about your current condition or injury
- Any recent accidents, surgeries or imaging
- Insurance and billing details (if applicable)
- Consent to treatment and acknowledgment of clinic policies
This information allows us to provide accurate, personalized care while maintaining a clear medical record for your ongoing treatment.
Insurance and Payment Options
We accept a wide range of insurance plans, including:
- Medicare

- Passport
- Personal Injury Protection (PIP) for auto accidents

If you have questions about coverage, please call our office before your appointment. We are happy to verify your insurance benefits or explain our self-pay rates if you are uninsured or not using insurance for your care.

Our front desk team is available to assist with any questions you may have about the paperwork or how to complete the forms correctly. If English is not your preferred language, we also offer bilingual support in Spanish to ensure full understanding and comfort throughout the intake process.
What to Bring to Your First Appointment
To help us provide efficient service, please bring the following to your first visit:
- Completed patient forms (or arrive early to fill them out)
- A photo ID
- Your insurance card (if using insurance)
- Any relevant medical records, X-rays or imaging reports
- A list of current medications (if applicable)
